Damen Dental Associates (formally West Lakeview Dental Group) has been providing quality dental care since 1983 when Dr. Lauterbach opened her doors at 1209 W. Diversey Pkwy. In 1988, Dr. Torma joined the group and in July of 2000, the practice moved to 1642 W. Belmont Ave. In July of 2013, Drs. Lauterbach and Torma welcomed Dr. Rachel Pappas to the group. In 2020, the practice moved to a brand new space at 4815 N. Damen Ave.

Insurance

We do accept most traditional PPO insurance plans and participate in-network with Delta Dental Premier. As a courtesy, we will handle all of the paperwork and make sure your claim is submitted to completion.

While our goal is to maximize your insurance benefits, it is important to understand that the insurance contract is between the insurance company and yourself or your employer. We can only “estimate” your insurance coverage. Dental insurance companies, unlike medical, rarely cover 100% of any dental fee. The treatment recommended by our doctors is never based on what your insurance company will pay but rather what they consider to be in the best interest of your oral health.

We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered. For your convenience, we accept Cash, Check, Visa, Master Card, Amex, and Discover. We offer payment options through Capital One’s popular healthcare credit card.
